What is Prostate Cancer?

The prostate is a small walnut shaped gland located just below the bladder. It supplies part of the seminal fluid (semen), which mixes & transports sperm from the testes.
Cancer is a disease in which cells in the body grow out of control. When cancer starts in the prostate, it is called prostate cancer.
Prostate cancer cells can travel through blood vessels or lymph nodes to reach other parts of the body. Such a condition is called metastatic prostate cancer.
No one knows why or how prostate cancer starts.

 

Symptoms of Prostate Cancer :

Prostate cancer may cause no signs or symptoms in its early stages. In its advanced state it may cause signs and symptoms listed below.   • Difficulty starting urination
  • Weak or interrupted flow of urine
  • Frequent urination, especially at night
  • Difficulty emptying the bladder completely
  • Pain or burning during urination
  • Blood in the urine or semen
  • Pain in the back, hips, or pelvis that doesn’t go away
  • Painful ejaculation

 

Screening for Prostate Cancer :

  • Cancer screening means looking for cancer before it causes symptoms. A blood test called a prostate specific antigen (PSA) test measures the level of PSA in the blood. PSA is a substance made by the prostate. A high level of PSA may indicate prostate infection, inflammation, enlargement or cancer.

Diagnosis of Prostate Cancer :

If you have a high PSA value during your screening, Dr Raghunath SK may recommend you to undergo the below test to determine if you have prostate cancer.

 

  • Transrectal Ultrasound : A probe is inserted into the rectum and high-energy sound waves (ultrasound) are bounced off the prostate to create a picture of the prostate.
  • MRI : Dr Raghunath may recommend a MRI, which will help him conduct and guide the biopsy procedure.
  • Biopsy : A biopsy is the main tool for diagnosing prostate cancer. During this procedure a small piece of tissue is removed from the prostate and looked at under a microscope to see if there are cancer cells.

 

Grading & Staging of Cancer :

  • One of the outcome of biopsy is the Gleason score. This determines how aggressively the cancer is spreading in your body. Score ranges from 2 - 10 , higher the score more fast it can spread.
  • Dr Raghunath will then determine how far the cancer has spread in the body. This is called staging of cancer. Depending upon the stage of cancer appropriate treatment plan will be recommended.

 

Treatment Options :

Dr Raghunath along with his team of medical & radiation oncologist will help you to determine your treatment plan based on multiple conditions like grade & stage of cancer, your age, your health conditions, etc. You will be explained various options. You & Dr Raghunath can together decide on the next course of action.

 

  • Active surveillance : Low-grade prostate cancer may not need treatment right away. Dr Raghunath will monitor your cancer through regular PSA test, DRE, & biopsies. Treatment to be started only if cancer grows or causes symptoms.
  • Surgery : Surgery for prostate cancer involves removing the prostate gland (radical prostatectomy), some surrounding tissue and a few lymph nodes. Dr Raghunath recommends robotic surgery and in some cases depending on patient condition, might opt for a open surgery. Click on this link to know more about Radical Prostatectomy.
  • Radiation Therapy : Radiation therapy uses high energy rays to kill cancer cells.
  • Chemo Therapy : Chemotherapy uses drugs to destroy cancer cells anywhere in the body. It is used for advanced stages of prostate cancer when cancer has spread to other organs. Often used with other treatment options.
  • Harmone Therapy : Prostate cancer cell need androgen (the male harmone) to grow. Harmone therapy is to reduces or suppresses androgen generation, thereby stopping the growth of cancer cells. This is done through medications and/or surgery.
  • Other Options : There are other options like Cryotherapy, Immunotherapy, HIFU/Focal therapy, etc.

 

Life After Treatment :

  • Once you have completed treatment, it is time to manage your possible side effects. Some men may have Urinary incontinence and/or Erectile Dysfunction for a few months. Dr Raghunath & team will help you to manage the same till it is resolved. You might have to come for follow-up check-ups.
